Case Studies

Northumbria Healthcare NHS Foundation Trust and Connect Health working in partnership to deliver Joint Musculoskeletal and Pain Service (JMAPS). 

Connect Health and Northumbria Healthcare NHS Foundation Trust came together to form a joint bid through recognising the strengths of the partnership whilst acknowledging the challenges of the current service. The partnership bid was successful and the JMAPS service commenced on 1st July 2019.

 

Working together to streamline services, cutting waiting lists and onward referrals in Brent.

In Brent we provide the Community Physiotherapy Service and Tier 2 Clinical Assessment and Triage Service (CATS) which provides a single point of access gateway for musculoskeletal (MSK) health, rheumatology, pain, women’s health, orthopaedics and diagnostics.

The Solution

Connect Health’s MSK services include the provision of triage, clinically-led multidisciplinary team assessment, diagnosis, treatment, rehabilitation and care planning for the local population.

This covers the assessment and treatment of muscles, tendons, ligaments, bones, spinal discs, nerves and other structures to help maintain and promote good MSK health.

We have an integrated end to end pathway from expert point of contact through FCP physiotherapy that realises benefits and efficiencies across the MSK pathway. This includes Tier 2 Clinical Triage and Assessment Services (CATS) which provides a single point of access gateway for musculoskeletal (MSK) health, rheumatology, pain, women’s health, orthopaedics and diagnostics.

We ensure timely consultation in the right place at the right time enabled through data and innovative care models in order to personalise care focusing on what matters to the person.
