Connect Health Health Coaching Services

We provide high value health coaching services using an asset-based approach delivered by accredited Health Coaches.

We understand Primary Care Networks, and how the new health coaching additional roles can work to enhance your existing teams, reduce pressure on practice staff and improve patient outcomes.

With a vision to enable our communities to live healthy and active lives, Connect Health’s strategy is to deliver positive change to models of care, to engage and empower people, by working together to build sustainable healthcare systems.

The Challenge

The Solution

Supporting Patients:

• Health coaching promotes supported self-management (skills not pills)

• Improved Patient Activation Measure® (PAM®)

• Improved physical, mental health and wellbeing outcomes

• Positive patient experience

• Reduction in the need for appointments

Supporting Integrated Care Systems:

• Financial savings – improvement of PAM® scores associated with an 8.3% reduction in ongoing health care costs

• Reduction in demand for services

• Positive impact on key outcomes such as self-efficacy, and health care usage

• New rapidly accessible and effective workforce

Supporting Primary Care:

• Decreased demand on services (up to 18% less GP appointments and 38% fewer A&E attendances)

• Upskilling and support for primary care

• Optimised use, including reduction, of medication

• Resources for implementing health coaching approaches amongst primary care staff
