What is the Patient Portal?

The Connect Health Patient Portal is our appointment information hub, providing you with access to useful tools throughout your journey of care with us.

* The managing appointments feature will only be available for certain patients and pathways at present, we will continually be enhancing this option as part of our Patient Portal development.

The Patient Portal is an online digital platform that allows you to access your own health records and take ownership of your care. You can self-serve to view documents and letters, complete questionnaires, and in some cases manage your appointment bookings.

You will sign in using your NHS login. These are the same credentials used to access other platforms like the NHS App.

Through the use of the Patient Portal, you will have more ownership of your care. You will be able to see the letters that are generated by clinicians within 48 hours, access any exercises assigned to you through Physitrack, view and amend appointments, request follow up appointments and more. It’s very simple to access and creates a much better-quality patient experience.

The Patient Portal is fully accessible for all users of the platform. It works with technology called Recite Me, which allows patients to use a screen reader to hear the content read out loud. This enables us to create an inclusive experience online by providing accessibility and language options to allow everyone to customise the portal in a way that works for them. It supports patients who are neurodiverse, visually impaired, speak English as a second language or are less confident in using digital technologies. Recite Me can translate the content on the portal into over 100 languages so it’s a fantastic tool to create an inclusive experience.
