What is Physitrack?

Connect Health use Physitrack, a web and iOS-based exercise platform, that allows clinicians to engage with patients around high-quality exercises and patient information. Patients can instantly access information via the PhysiApp patient portal or downloadable app, where they can engage visually with a clinician, watch video demonstrations of their prescribed exercises and record compliance, enhancing patient outcomes and improving choice.

Physitrack is designed to be user-friendly. Patients can set notifications to do their exercises at a time that is convenient for them or as prescribed. Results can be monitored and tracked in real time, and provide a useful tool to aid patient engagement and assessment.

 

Can Physitrack see my medical records and personal data?

Physitrack meets all NHS required data protection standards including DTAC and the Data Security and Protection Toolkit, and are fully compliant with GDPR in regards to the Data Protection Act 2018. Physitrack’s data servers are UK-based to comply with NHS Trust regulations and the platform only requires the patient’s name and year of birth to set up a programme.
