If you currently do not have an appointment scheduled but have been a Connect Health patient in the last 6-months and require further support, click here.

Connect Health will provide the service up to Thursday 28th March 2024.

The new provider, Circle Integrated Care, has been awarded the service contract by NHS Hertfordshire and West Essex Integrated Care Board (HWE ICB), the organisation responsible for the planning and buying of local NHS health services.

There are plans in place between Connect Health, Circle Integrated Care and NHS HWE ICB to ensure that there is continuity of care for any patients currently receiving treatment, who need to be transferred over to the new service.

You can contact Circle Integrated Care on 01442 500 985 after 1st April 2024.

Your care records detailing your therapy treatment to date with Connect Health will be transferred securely to Circle Integrated Care (in accordance with the Data Protection Act 1998). However, your full care record will be retained by Connect Health and will remain accessible to you for a period of eight years.

Any future records for patients assessed and treated in the new service after 1st April 2024 will be created by Circle Integrated Care, who will become the data controller.

There is no need to contact Connect Health if you wish to continue your treatment, this will be provided by Circle Integrated Care from 1st April 2024.

Your records will be shared with Circle Integrated Care allowing clinicians in the service access to your previous clinical information.

You will be contacted by Circle Integrated Care in due course to arrange a suitable appointment after 1st April 2024.
