Connect Health will provide the service up to Friday 30th September 2022

From 1st October Vita Health will be providing the Physiotherapy service and Croydon Health Services NHS Trust the Clinical Assessment and Treatment Service (CATS)

If further treatment is required your care and electronic records will be transferred to the relevant service. Vita Health for Physiotherapy and Croydon Health Services NHS Trust for CATS.

The new providers will be in contact with all patients that have transferred across to them.  They aim to have contacted everyone to arrange an appointment within the first 2-3 weeks of October.  If you have not heard from them by the 21st of October you can contact them on:

Connect Health will share your records with the new provider, so the clinicians will have access to this information. Your clinician will likely need to revisit specific parts of your history or treatment to date to ensure best possible care is provided.

Contact your GP who will be able to arrange a referral to the new providers.

Urgent patients are prioritised and we do have some limited appointments available to book before the end of September. Referrals are triaged by Advanced Practice Clinicians or via PhysioNow digital assessment tool. If your referral is identified as urgent Connect Health will attempt to make an appointment for you.

Routine Physiotherapy and Clinical Assessment and Treatment Service (CATS) appointments are now fully booked until the end of September. If a new referral is received during this time your care will be transferred to the new providers.
