Connect Health have been asked to manage and deliver the community physiotherapy service on behalf of Brent CCG. Our aim is to build on the success of the previous service through application of our specialist MSK supportive infrastructure.

We expect the service to develop and become larger and as such are looking for skilled, motivated and passionate clinical staff to join the existing team in delivering a fantastic, innovative local service to the people of Brent. You will work in conjunction with the current staff to join and enlarge their team.

Our philosophy of care is based on patient empowerment; ensuring all people have timely access to the right information, allowing them to make informed, shared decisions about their future care needs. We support staff to understand the importance of a Biopsychosocial (BPS), evidence-informed approach which has shared decision making at its heart, as well as a focus on activity/exercise.

We are looking for motivated APP’s who are passionate about the consistent delivery of quality clinical management of musculoskeletal conditions. We are looking for generalist APP’s or those with a specialist interest in specific body-parts and skill-sets (e.g. Ultrasonography). We would also be keen to discuss supporting APP’s to develop the above areas of expertise.

You’ll work as a self-sufficient practitioner and deliver a quality clinical service meeting the clinical and commercial needs of the service. The role will incorporate clinical triage ensuring appropriate sign-posting of patients internally or externally to best meet their continued care.  The direct patient-facing component of the role will entail the undertaking of a comprehensive assessment and diagnosis of patients with a wide variety of physical and emotional needs.  You will formulate, deliver, review and progress individual programmes and ensure effective treatment, specialist advice and timely discharge is achieved.

Developing and promoting a positive and dynamic image and culture, you will be key in fostering productive relationships with key stakeholders such as local Consultants and GP’s, and will provide support and training to other clinical staff and students.
